Résumé

Working in the healthcare sector is under constant pressure. The nursing profession, already burdened by the complexity of care, comorbidities, and an aging population, has undergone numerous transformations in recent years. Recruitment challenges have added to the workload. There are more patients, shorter hospital stays, and consequently, more notes to write—notes that must be thoroughly documented and often written more quickly to keep up with demands. I’ve been a nurse for 16 years. I have always been concerned about the well-beingof patients and the quality of practice. I agreed to review Mireille’s guide for a secondtime to help improve the practice of chart notes. Chart notes reflect the qualityof our assessment, clinical reasoning, and judgment. It is therefore importantthat they are complete, accurate, relevant and clear. Unfortunately, this is an areathat is all too often overlooked, yet it is so important for our own safety and thatof our patients. I hope you enjoy reading this guide and that it meets your expectations.Kim Willcocks, Nurse Practitioner
